What do I need to include in my Gems claim?

  • Your Gems membership number.
  • The scheme for which you have signed up.
  • Your Surname and Name.
  • The name of the patient and their beneficiary code. You can get this on your membership card.
  • Name and practice number of the place where you were treated.
  • Date of treatement.
  • Proof of payment to the medical services provider. This can be in the form of a receipt or an EFT receipt.
  • Tariff code.
  • The ICD-10 code.
  • Your banking details also need to be included in your claim. That includes your account number, the bank’s name and branch code and the account holder’s name.
  • Your signature.

    What form do I need to complete when I make a claim.
    I paid the doctor already and need to claim from Gems. But if open Gems
    forms on the website, I do not find the form to fill in and send with my proof of payment.
    There are international claim forms but no normal claim forms.
    If someone can assist, please.
